At times, a leader will notice a recurring pattern of underwhelming results. That may mean there is some kind of below-the-surface attachment to the status quo. Asking "Why does this keep happening?" leads only to symptoms -- not root causes. Instead, try asking, "What's at stake if I keep things the way they are?" Take that answer, and ask again: "Okay, now what's at stake?" Repeat the question until you can go no further and you are likely to discover the source of the enduring problem. Solutions will then become evident, helping you make the changes necessary to break the pattern and improve overall effectiveness.
